How to apply for your child's passport | Motherhood Unplugged

All children must have a passport in order to travel internationally. Minors under the age of 16 need to apply for a children's passport.
What you will Need to Apply:
1. Completed DS-11form
2. One 2x2 passport photo ( I have taken mine at Walmart + CVS)
3. Evidence of child's US Citizenship (birth certificate or passport)
4. Evidence of relationship between child and parent or guardian (birth certificate, adoption decree, divorcee/custody decree)
5. Parent or guardian's ID + a photocopy
6. Provide parental consent (in the case of one not being able to be present you can have a notarized form signed by that party)
7. Application fee

Frequently asked questions:
Can I mail my child's application in?
No, children applicants under 16 are not eligible to mail in their passport applications. Additionally, the Department of State does not currently have the option to submit passport applications online.
Do I need to make an appointment?
Some facilities require appointments and others do not, I would suggest calling the place you wish to go ahead of time. Also note that the warmer months seem to be busier.
